FAQ 119 Why have addresses and phone numbers been removed from Witness Testimony forms?

Answer 119;

During the Recording Secretary training session in March 2026, it was recommended that witness’ addresses and telephone numbers should no longer be included on pages 2 and 3 of the Witness Testimony documents. This change follows reported incidents where witnesses faced severe threats, including intimidation at gunpoint and threats to their personal safety.

To protect the health, safety, privacy, and well-being of witnesses, conveyed/reconveyed living men and women are strongly advised not to disclose the home addresses or telephone numbers on these forms.

NOTE :This recommendation does not apply to the witness shown on the first page of the Witness Testimony document.
